Hi! I would like to request VaxCert as I have an upcoming travel this August 21, 2023.
I have an existing request with reference number 4332653 but was rejected.
I have submitted all the needed documents - scanned copy of the following
1. Vaccination Card
2. UMID ID
3. Travel Authority
Moreover, since my initial request was rejected I have filed another request with Reference number 4334235.
For information, I have emailed the vaxcertsupport@doh.gov.ph in 2021 and this July 2023 but to no avail.

Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate.
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.

This is to inquire of the LGU where my vaccination record for me to be able to file request for assistance to generate my vaccination certificate. The VAXCertph is still unable to address my request. Please see attached screen shot of the site.
