Hi. I recently tried to retrieve my vaccination record through https://vaxcert.doh.gov.ph, but I received a notification indicating that no records were found under my name.

The following are my vaccination details:
Full name: Gelli B Campos
1st dose - 08/04/2021
2nd dose - 09/01/2021
Additional Dose (Booster) - 01/11/2022
City/Municipality: City of San Fernando La Union

I would greatly appreciate it if you could respond to my request ASAP as I will be travelling tuesday, July 25,2023. Thank you for your kind and immediate assistance.

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
Rest-assured that we've already coordinated with Agent 162 from the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435. Please expect a call or an e-mail from them, if none, kindly call them back through the number above. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
