To whom it may concern,

I am REIGN JULIUS CERIA, a seafarer who will go onboard this First Week of August. I verified my Vaccination Certificate and found out that my 2nd dose does not exist. I have requested a certification from our LGU to which I had my 2nd dose of vaccination.

I am requesting a VaxCert for my second dose to have my appointment of yellow card for Covid19 at the Bureau of Quarantine which shall be on Monday (July 31, 2023), with the following details:

Type of Vaccine: Astrazenica
1st dose: 11/25/2021 (CTMAV596)
2nd dose: 01/07/2022 (PW40171)

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
Rest-assured that we've already coordinated with Agent 243 from the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435. Please expect a call or an e-mail from them, if none, kindly call them back through the number above. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
