Hi, good day ma'am/sir gusto ko lang po sana mag request ng certificate of vaccination, ako po si Lucille Anne V. Balog, 18 years old, noong july 2,2022 po ang first dose ko at ang tinurok po sakin ay P-fizer ako po ay nagpa vaccined sa Ragay, Camarines Sur, nawala po ang vaccination ko sa bus terminal nalaglag po ito at hindi ko na nahanap, kaya ngayon nagrerequest po sana ang ng vaccination Certificate sa katunayan na ako ay fully vaccinated na, kailangan ko po kasi ito ngayon sa college para po makaenroll ako, thank you po

About the lost vaccination card, kindly coordinate
with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
