Good day Ma'am/Sir!

I would like to request for a cancellation of my vaxcert update request created on July 22, 2023. I noticed that there is an error on the date that inputed on the second dose on the site when I compare it to my vaccination card. Thus, I would like to request again once the prior was cancelled.

You asked for Cancellation of Vaxcert Update Request.

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records and/or cancellation of request is done by the concerned vaccination center. If your vaccination record is unavailable, the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
If you need to cancel request, you may just diregard it or kindly coordinate with the LGU where you received your vaccination and request for assistance to let you print her vaccination certificate.

Your right to request a review
