Please be informed that the application for Certified True Copy of Derogatory Records is only accepted and processed at the head office of the Bureau of Immigration, Intramuros, Manila for reasons of security and uniformity.

A Certified True Copy of Derogatory Records may be issued upon submission of the following requirements:

1. The Letter request addressed to the Commissioner;
2. Duly accomplished application form which may be downloaded at https://immigration.gov.ph/images/FORMS/FY_2019/Certification/CCS-A-CDR-2016-CertiedTrueCopy_DeroRecords.pdf or may be accomplished at BI main offce;
3. Special Power of Attorney (SPA) from applicant with a photocopy of a valid government-issued ID of attorney-in-fact in case the application is filled by an authorized representative;
4. Photocopy of applicant’s passport bio-page;and
5. Processing Fees - Php 1,010.
