To whom it may concern,
I am Racquel C. Arabos from the town of Binangonan, Rizal. I would like to request a BOOSTER SHOT CERTIFICATE from your good office for travel use purposes to enter and show a valid authenticated certificate to Saudi Arabia country. I got my Booster shot from Antipolo, Rizal Pfizer-BioNTech batch and lot no. PCA0008 last January 3, 2022.For the reason that they allowed only 4 vaccine shot for their country including Pfizer.
I am hoping for your kind consideration and response to my request thank you in advance and God Bless.

Response to your request

While our aim is to provide information whenever possible, in this instance this Office could not provide your request for issuance of Booster Shot Certificate or vaccination certificate. However, you may wish to contact Department of Health (DOH) doh.gov.ph, or resubmit your request addressed to the correct agency on the eFOI portal at their website.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to the BI Commissioner, JAIME H.
