Good day!
 
I would like to inquire regarding about a lost immigration arrival sticker.l arrived last October 3,2023 and it's been 9days now since I've notice that the sticker was not attach in my passport and I couldn't find it anymore.i would appreciate your immediate feedback.thank you!

I refer to my letter of Oct 13, 2023 12:15:58 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for arrival sticker.

In order for us to work further on your request, we need more information from you regarding your query. Please kindly provide the following information:

a. Name appearing in the passport;
b. Date of Birth;
c. Passport Number;
d. Date and Time of arrival;
e. Port of Entry (Airport);
f. Flight Number; and
g. Airline.

In particular, it would be helpful to know the above mentioned information.
