Good day Sir/Madame, 
I recently moved back to the Philippines and have lost my copy of the Oath of Allegiance. Can I request for a copy please. I took my oath in Honolulu, Hawaii in May 2020. Thank you so much for your help. If you need any proof of identification or any documents from me, please contact me at this email. Thank you again for your time.

Response to your request

While our aim is to provide information whenever possible, in this instance this Office does not have the information you have requested. However, you may wish to contact Department of Foreign Affairs (DFA) at dfa.gov.ph and for the request for an appointment relative to your request for a copy of your Oath of Allegiance which was made in Philippine Embassy. 

We are respectfully extending our apologies because query and/or request is not within the Bureau’s jurisdiction.

Your right to request a review
