Dear Immigration Office,

I am writing to seek guidance on the process and required documents for replacing my father's lost Philippine passport.

My father was born in 1954 to a Chinese father and Filipino mother, elected Filipino citizenship at the age of 21. He previously held a Filipino passport, but unfortunately, it was lost around 30 years ago.
Could you please provide detailed information on the necessary steps and documents involve in obtaining a replacement passport for him? Thank you very much for your time and assistance.
