Hi 

My passport details are not matching to my vaccine certificate. Please help. The vaccination center didn't accept my passport at the time vaccination. So I had to go ahead with the documentation I had at that time. Now this year may I am planning to go on a holiday to Singapore along with my sister's family. But worried that my passport details are not matching to my vaccine certificate. Please help

With regard to your concern, kindly coordinate with the Department of Health and the Department of Foreign Affairs as both agencies could appropriately address your inquiry. 

Your right to request a review
