Hi,

I would like to request about my travel history 10 years ago as it will be an important document for the application of visa. I am an overseas Filipino worker have worked in Laos. I have lost my previous passports. Please allow me to grant my request. Hoping for your kindness accomodating this request.

Your Request

You asked for Travel History.

Response to Request

Your FOI request is approved. In order for us to issue you a travel certificate, you need authorize someone in the Philippines to process your application. The requirements are the photocopy of your passport's biopage, authenticated SPA from Philippine Embassy in Laos, photocopy of ID of your representative and application form from BI.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Patricia Flores CC Section. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.

If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
