One of the documentary requirements for request for travel records is the 'Photocopy of Applicant's Passport Bio-page, Departure and Arrival Stamps for the Period Requested' (item 2 of the checklist, BI FORM 2014-13-005 Rev 0). In the absence of such a requirement, what document must I bring with me to the Bureau of Immigration Head Office to avoid denial/delay in the processing of the request for travel records?

Dear Shez Gilleine Castillano,

Greetings!

Thank you for your request dated Jun 06, 2023 04:16:12 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Please be informed that the application for Travel Records are only accepted and processed at the head office of the Bureau of Immigration, Intramuros, Manila for reasons of security and uniformity.

If you are unable to personally process the application, you can designate a representative by providing an appropriate Special Power of Attorney (SPA). In the absence of a passport, you may bring any government issued primary Identification Card like driver’s license, National ID or UMID.

Further, you may opt to choose either of the two processing procedure, Regular (15 working days, Php510.00) or Express (7 working days, Php1, 010.00).

Attached is a copy of the checklist of requirements for your reference.
