Hi. I would like to request for my IC license number or why I have not received mine. I submitted my license application thru Axa Philippines last August 30, 2022.

Greetings!

Thank you for your request dated Sep 21, 2022 11:38:13 AM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Your Request

You asked for Insurance Agent License.

Response to Request

In relation to your request, please be advised that you may visit the Licensing Division  or send an email at licensing@insurance.gov.ph to request for a Certification of Agent's License. In order to process the said request, you are required to: 1) Set an appointment with the Insurance Commission-Head Office through the  IC Appointment Reservation System which can be accessed through https://web.insurance.gov.ph/icare/login ; 2) submit a request letter indicating  your complete name  (First Name, Middle Name and Surname), and 3) to pay a certification fee in the amount of Five Hundred Pesos (Php500.00).  Please also be informed that you will be required to submit a copy of your government-issued identification card for verification.

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by email at rr.mojica@insurance.gov.ph. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.

If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
