Hello Sir/ Madam, 

I have passed the IC exams to sell insurance years ago (but I could not remember exactly when); however I went on to work in the government so I did not continue in the industry. Before COVID, I decided to retire early and now I plan to return to selling insurance. In this wise, may I respectfully request if my license then is still valid as of today. Thanks very much!

Dear Mr. Ignacio, 

Thanks so much Sir for the prompt reply. I look forward to your findings re my license SOON so I would know whether or not I need to take the (insurance) examinations anew. 

If you may please extend your search for "REGIN RAYMUND D. DAIS" back to 2010. If it would help, I also remember taking the exams under the aegis of then PhilamLife (now AIA Philippines). 

I have also attached an ID of mine for your reference.

Do you already have info on my request Sir re validity of my license as insurance agent before? My apologies for the follow-up but I am praying for an earlier info ahead of the next licensing exams of your agency on May 20. So that, if my license is still OK, I can save on the examinations fee.

You asked regarding to your insurance license.

Response to Request

In relation to your request, please be advised that you may visit the Licensing Division  or send an email at licensing@insurance.gov.ph to request for a Certification of Agent's License. In order to process the said request, you are required to: 1) Set an appointment with the Insurance Commission-Head Office through the  IC Appointment Reservation System which can be accessed through https://web.insurance.gov.ph/icare/login ; 2) submit a request letter indicating  your complete name  (First Name, Middle Name and Surname), and 3) to pay a certification fee in the amount of Five Hundred Pesos (Php500.00).  Please also be informed that you will be required to submit a copy of your government-issued identification card for verification.

For further inquiries, you may contact our Licensing Division at 8523 8461 local 111.

For your information and guidance.

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by email at icrecords-foi@insurance.gov.ph. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.
