Dear Ms. Bernadette Capisonda,

Greetings!

Thank you for your request dated Feb 01, 2020 09:43 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You requested for a copy of your examination results taken last June or July 2017 at the PhilamLife Office in Pampanga.  

We understand that the said data/information will be used in your application as insurance agent of another insurance company. Moreover, based on the records of the Insurance Commission, you have been authorized to act as insurance agent of the Philippine American Life and General Insurance Company. With this, may we inform you that, in applying for an agent's license of another life insurance company, a copy of examination result is no longer required provided, that a certificate of clearance has been issued by the authorized company representative of applicant's previous employer. However, should you still wish to obtain a certification on the result of your qualifying insurance agent's examination, you may do so by submitting a formal request letter, and payment of  Php500.00 certification fee.

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to te.espejon@insurance.gov.ph. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.

If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
