I refer to my letter of Nov 11, 2021 01:54:45 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Registration and License to Act.

Your Request

You asked for Registration and License to Act.

Response to Request

In relation to your request, please be advised that, based on the records of this Commission, Inspira Life Insurance Agency Corporation has been issued an agency license with validity from 26 February 2019 to 31 December 2021.

You may secure a Certificate of License from this Commission through the submission of a formal request letter to the Licensing Division. Also, kindly prepare a certification fee in the amount of Five Hundred Pesos (Php500.00).

Moreover, please be reminded of the following: 1) Setting of appointment thru the IC Appointment System which can be accessed through https://web.insurance.gov.ph/icare/login ; 2) Office hours of the Commission is from 9:00 A.M. to 4:00 P.M. only, and 3) Cut off time of our Cashier Section is until 2:00 P.M. only.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by email at rr.mojica@insurance.gov.ph. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.

If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
