You asked for License Number.

Response to Request

In relation to your request, please be advised that based on the records of this Commission, you are not licensed to act as an insurance agent by this Commission. 

Your applications for traditional and variable life insurance Agent’s License has been cancelled by Philippine AXA Life Insurance Corporation, through a letter dated 20 April 2022, due to non compliance with lacking requirements.

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by email at rr.mojica@insurance.gov.ph.
