Hi. May I have a proof of certificate of both Trad and VUL. Here is my license no. 10008766.

How long does it take before the license expires? Should I renew it every year / every 3 years? They told me that my license is expiring at the end of this year. I'm not an active agent since I work 2 jobs. I'm not making a sales for about 6 months. May I know if how long does my license if effective?

You asked for Proof of Certification and Query.

Response to Request

In relation to your request, please be informed that based on the records of this Commission, you have been authorized to act as Life and Variable Insurance Agent of FWD Life Insurance Corporation issued on 14 May 2021 valid until 31 December 2023.

Should you wish to request Certificate of License, kindly submit a formal letter request to IC Licensing Division together with the payment of certification fee of PhP500.00.
 
Please note that this Commission can only issue a Certification for examination administered by this Commission.

Moreover, please be reminded of the following: 1) Setting of appointment thru the IC Appointment System which can be accessed through https://web.insurance.gov.ph/icare/login ; 2) Office hours of the Commission is from 9:00 A.M. to 4:00 P.M. only, and 3) Cut off time of our Cashier Section is until 2:00 P.M. only.

Your right to request a review
