This is to request IC certificate of License for VUL and Traditional, as this is required for the issuance of Professional Tax Receipt in Manila City Hall. I am currently affiliated with Sun Life PH.

Greetings!

Thank you for your request dated Nov 23, 2021 09:55:01 AM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Your Request

You asked for IC certificate of License for VUL and Traditional Insurance.

Response to Request

In relation to your request, you may visit the Licensing Division at the IC Head Office located at 1071 United Nations Avenue, Ermita, Manila. In order to process the said request, you are required to: 1) Set an appointment with the Insurance Commission-Head Office through the  IC Appointment Reservation System which can be accessed through https://web.insurance.gov.ph/icare/login ; 2) submit a request letter indicating the date, place, and the type of examination taken, and 3) pay a certification fee in the amount of Five Hundred Pesos (Php500.00) at the Cashier Section.

Please note that this Commission can only issue a Certification for examination administered by this Commission.
