Your Request

You asked for Prulife Uk Insurance License.

Response to Request

In relation to your request, please be informed that you may secure a Certificate of Agent’s License from this Commission through the submission of a formal request letter to the Licensing Division. The request letter should include the type of license being requested for certification, and the company being represented. You must also prepare a certification fee in the amount of Five Hundred Pesos (Php500.00).

Moreover, please be reminded of the following: 1) Setting of appointment thru the IC Appointment System which can be accessed through https://web.insurance.gov.ph/icare/login ; 2) Office hours of the Commission is from 9:00 A.M. to 4:00 P.M. only, and 3) Cut off time of our Cashier Section is until 2:00 P.M. only.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by email at rr.mojica@insurance.gov.ph. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter.
