Greetings!

Thank you for your request dated Sep 19, 2021 08:31:47 AM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Your Request

You asked for Verification of Insurance License Number.

Response to Request

In relation to your request, please be informed that according to the records of this Commission, you have active licenses as an insurance agent and thus, may secure a Certificate of Agent’s License from this Commission through the submission of a formal request letter to the Licensing Division. 

The request letter should include the type of license being requested for certification, and the company being represented. You must also prepare a certification fee in the amount of Five Hundred Pesos (Php500.00).

For the process of agent license renewal, kindly coordinate with the company you are representing. You must inform them of your intent to renew your existing agent licenses as processing of applications for agent license renewal is being handled by and coursed through the  insurance company that an agent represents.

Moreover, please be reminded of the following: 1) Setting of appointment thru the IC Appointment System which can be accessed through https://web.insurance.gov.ph/icare/login ; 2) Office hours of the Commission is from 9:00 A.M. to 4:00 P.M. only, and 3) Cut off time of our Cashier Section is until 2:00 P.M. only.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by email at rr.mojica@insurance.gov.ph. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.

If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
