May I request to your good office a certification of my examination in VUL and TRADITIONAL last 2013? I took the licensing exam march 26,2013 for VUL. The Traditional exam i didn't remember the exact date when.

Your Request
You asked for Certification of Examination.
Response to Request
In relation to your request, you may visit the Licensing Division at the IC Head Office located at 1071 United Nations Avenue, Ermita, Manila. In order to process the said request, you are required to: 1) Set an appointment with the Insurance Commission-Head Office through the IC Appointment Reservation System which can be accessed through https://web.insurance.gov.ph/icare/login ; 2) submit a request letter indicating the date, place, and the type of examination taken, and 3) pay a certification fee in the amount of Five Hundred Pesos (Php500.00) at the Cashier Section.
Please note that this Commission can only issue a Certification for examination administered by this Commission.

Your right to request a review
