Hi! Magandang araw po sa ating lahat.
Ako po ay nanawagan sa Freedom of Information patungkol po sa CLAIMS sa PHILHEALTH ng aking yumaong Nanay noong Hulyo 13, 2020 sa diagnos na sakit ng critical pneumonia at lumabas din ang result na sya ay COVID-19 positive.  Ang kanyang pangalan:Lydia Cantos Bartolome

I-finile po namin ang nasabing claims noong Sept28, 2020.  Maari sana naming i-follow up ito sa kadahilanan na ibabalik din namin sa sinanglaan namin para makuha namin ang aking Ina sa MMC hospital na kung saan ay hineld up po nila ang bangkay dahil wala kami pambayad sa ospital. Salamat po

Your FOI request is approved. I enclose a copy of [most] of the information you requested.

Upon verification the claim was Returned to Hospital (Medical Center Manila) for submission of the following;

1. No attached Official Receipt or Waiver
2. Original PhilHealth Claim Form 2 Not Properly Accomplished
3. Required Medical Documents from Medical Center Manila

Kindly coordinate with the said Health Care Institution for the compliance of the said requirements to facilitate the processing of the claim.
