To whom it may concern:
 
I would like to request the following information:

-detailed breakdown of expenses (showing the cost of materials, machines, labor etc) of PhilHealth in producing and issuing the PhilHealth ID/UMID card from the year 2010 to 2016.

-the current price of acquiring an ID Card per individual.

We would highly appreciate if the files you have would be in a .xls format but if this is not available, a .pdf format would suffice.
