Thank you for your request dated Mar 07, 2023, 03:48:23 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Your Request

You asked for Voluntary Contribution.

Response to Request

If you want to change your PhilHealth contribution from employed to voluntary, you will need to follow these steps:

1. Visit the nearest PhilHealth branch or satellite office. You can find a list of PhilHealth offices on the PhilHealth website or by calling the hotline (02) 8441-7442.

2. Secure a PhilHealth Member Registration Form (PMRF) and PhilHealth Member Data Record (MDR) from the PhilHealth branch or satellite office. You can also download the forms from the PhilHealth website.

3. Fill out the PMRF and indicate that you want to change your membership category from employed to voluntary.

4. Submit the completed PMRF along with a photocopy of any valid ID (e.g., passport, driver's license, voter's ID) to the PhilHealth representative.

5. Wait for your MDR, which will reflect your updated membership category and contribution amount.

6. Pay your new contribution amount at any PhilHealth office or accredited payment center.

7. Keep your MDR and receipt of payment for your records.

It's important to note that if you are currently employed and covered by your employer's PhilHealth membership, you must secure a Certificate of Employment (COE) indicating that you are no longer employed and therefore not covered by your employer's PhilHealth membership. You will need to submit this COE along with your PMRF when applying for voluntary membership.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to <Emmanuel R. Ledesma, Jr. - Acting President and Chief Executive Officer (APCEO), nrsantiago@philhealth.gov.ph, or privacy.dpo@philhealth.gov.ph>. 

Your review request should explain why you are dissatisfied with this response and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.

If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
