Response to Request
While our aim is to provide information whenever possible, In this case, we will be able to provide the information requested because PhilHealth member information is classified as confidential information, we suggest you send an email to actioncenter@philhealth.gov.ph regarding your request. In your email please indicate the following information: PhilHealth Identification Number (PIN), Complete Name (Surname, First Name, Middle Name), Birthdate, Birthplace, Address, SSS or TIN No. (if any), Name of Dependents and their Birthdate. Also, attach a properly accomplished PhilHealth Member Registration Form (PMRF), and two (2) valid IDs. Kindly read the instructions at the back of the form for other supporting documents or you can also visit the nearest PhilHealth offices.
If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to <Emmanuel R. Ledesma, Jr. - President and Chief Executive Officer (PCEO), nrsantiago@philhealth.gov.ph, or privacy.dpo@philhealth.gov.ph>.
Your review request should explain why you are dissatisfied with this response and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.
If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
