Thank you for your request dated Jan 21, 2022 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You asked for Philhealth.

Response to your request

Please be informed that you may now register at PhilHealth Online Service - Member Inquiry where you can view your record, monitor your remitted/posted contribution, and print your MDR https://memberinquiry.philhealth.gov.ph/member/ .
Also, you may pay your contribution online thru this account, you just have to make sure that you have declared your monthly income at the nearest Local Health Insurance Office (LHIO in your area to generate your SPA.

Be reminded of the following in your Member Inquiry's registration: 

1. You have to register again or CREATE ACCOUNT because due to the enhancement, previously created accounts are automatically deactivated; 
2. If you have letter ñ in your name and/or last name, kindly use ALT 164; 
3. "One email per registered account", if email already used, kindly register using a different email address for your PhilHealth number; 
4. If encounter an error, kindly register using other browser; 
5. Don’t include your suffixes name like Jr, Sr., I (numerical number), or try to not include period in your name like MA instead of MA. and 
6. If error is still occur despite following the above reminders, kindly send us the screenshot or print screen of the Basic Information part with entry and we will endorse it to the proper office. For complete details regarding Member Inquiry, you may follow this link:

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to actioncenter@philhealth.gov.ph. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.

If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
