Hi! I would like to request information about the following:

1. What are the needed requirements for replacing PhilHealth ID? 
2. Can I use birth certificate and nbi clearance if I don't have any other valid ids? 
3. Can I replace my id at any office/branch of PhilHealth?

You asked for a Replacement ID for PhilHealth.

Response to Request

To get a new PhilHealth ID, the requester may submit the accomplished PMRF with the attached (1) valid ID at the nearest PhilHealth Office.

Your right to request a review
