While our aim is to provide information whenever possible, in this instance this Office does cannot provide the information you have requested due to Data Privacy Act. However, you may send an email to actioncenter@philhealth.gov.ph indicating the below information:
1. Complete Name (Last Name, First Name, Middle Name)
2. Birthdate & Birthplace
3. Address
4. SSS No. / TIN No. (if any)
5. Current or Previous Employer
6. Name of Dependent (if any)
7. Attach a copy of Valid ID

You can also send your request thru our Callback Channel.  Just text "PHIC callback <space> your mobile number or Metro Manila landline" (Strictly no other details) to 0917-898-7442 and we will call you back during office hours on weekdays.  (Note: Calling this number is discouraged.  If not accommodated due to high volume, please resend after 24 hours.)

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to actioncenter@philhealth.gov.ph. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.

If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
