April 6, 2018 Dear Christian, Greetings! Thank you for your request dated Apr 05, 2018 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your request You asked for Certified True Copy of One (1) Statement of Account of Patient with corresponding case rate of RMIU. Response to your request While our aim is to provide information whenever possible, in this instance the information you have requested must have authorization to retrieve a record from a previous patient of the said Hospital.
