Response to Request

Your FOI request is approved. I enclose a copy of [all] of the information you requested. In order to protect the personal data/information of PhilHealth members, PhilHealth encourages the requestors to get the requested information personally through our nearest PhilHealth branches/offices. The requestor may submit the accomplished PMRF with the attached (1) valid ID.

Your right to request a review
