Response to your request We were able to determine that this particular information is already available online and therefore cannot be counted as a valid FOI request. You may retrieve your requested information from https://www.philhealth.gov.ph/unclaimedrefunds/. In addition for verification of your claim, we would suggest you to send us an email with the following details for validation of the claim, 1. PhilHealth Identification Number (PIN) 2. Name of member (Last Name, Middle Name, First Name 3. Birthdate 4. Name of Hospital / Facility 5. Name of Patient 6. Confinement Period. You may send it to actioncenter@philhealth.gov.ph. You may also try PhilHealth's Callback Channel: 0921-630-0009 Just text: "PHIC callback (space) your Landline or Mobile Number” (strictly without other details) to 0921-630-0009 and we will call you back during office hours on weekdays Your right to request a review If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to actioncenter@philhealth.gov.ph. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request. If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011). Thank you.
