Hi i would like to request to replace my lost philhealth id? i dont have any valid id, except my birth certificate and brgy id.

can i go to the nearest branch and request for a new id?

You asked for Lost Philhealth ID.

Response to Request

 To get a new PhilHealth ID, the requester may submit the accomplished PMRF with the attached (1) valid ID at the nearest PhilHealth Office.

Your right to request a review
