Thank you for your request dated Jan 23, 2023 06:39:10 AM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Your Request

You asked for PhilHealth Registration and PhilHealth ID by authorization.

Response to Request

Yes, you can authorize someone else to register you for PhilHealth and collect your PhilHealth ID on your behalf. In order to do this, you will need to provide a signed authorization letter that includes the following information:

1. Your personal information, such as your name and address
2. The name and personal information of the person you are authorizing
3. A statement that you are authorizing the person to register you for PhilHealth and collect your PhilHealth ID

It's also good to include a copy of your ID for the person you authorize.
It is important to note that, the person you authorize must also bring a valid ID for presentation and verification purposes.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to <Emmanuel R. Ledesma, Jr. - Acting President and Chief Executive Officer (APCEO), nrsantiago@philhealth.gov.ph, or privacy.dpo@philhealth.gov.ph>. 

Your review request should explain why you are dissatisfied with this response and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.

If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
