Good day!

I am required to send MDR form to ny new Employer as part of my Pre-employment requirements.

Hope you could grant my request for this is one of the remaining requirements I need to comply.

Kindly indicate the following information on your email 1. PhilHealth Identification Number (PIN) 2. Complete Name (Surname, First Name, Middle Name) 3. Birth date 4. Address 5. SSS No. / TIN No. 6. Current / Previous Company. 7. Name of Dependent (if any). Attached a fully accomplished PhilHealth Member Registration Form (PMRF) https://www.philhealth.gov.ph/downloads/membership/pmrf_012020.pdf and two (2) Valid ID's.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to actioncenter@philhealth.gov.ph.
