While our aim is to provide information whenever possible, in this instance this Office does cannot provide the information you have requested due to Data Privacy Act. However, you may send an email to actioncenter@philhealth.gov.ph or message our official FB page at https://www.facebook.com/PhilHealthOfficial indicating the below information:
1. Complete Name (Last Name, First Name, Middle Name)
2. Birthdate & Birthplace
3. Address
4. SSS No. / TIN No. (if any)
5. Current or Previous Employer
6. Name of Dependent (if any)
7. Attach a copy of Valid ID

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to actioncenter@philhealth.gov.ph.
