Good day, I would like to ask regarding my father's Philhealth who died on Oct. 23, 2023, 44 years of age died due to Cancer Stage 4.I am 19 years of age and I am always the one who manages papers regarding my father's hospitalization and even during his burial. 

1. I would like to ask if I have to report on the Philhealth about his death or just leave it be?
2.  what should I do regarding his Philhealth?
3. Can we still use his Philhealth? 
4. And is there any death claim in Philhealth?

Your Request

You asked for an inquiry about the PhilHealth membership of your deceased father 

Response to Request

Your FOI request is approved. I enclose a copy of [all] of the information you requested. In case of the death of member, qualified dependents of the deceased member shall continue to be entitled to benefits availment for the remaining unexpired portion of the coverage period. To ensure eligibility of dependents of deceased members beyond the coverage period, the surviving spouse shall accomplish and submit a PhilHealth Member Registration Form (PMRF) to the nearest PhilHealth office to register as principal member and transfer the qualified dependents under his/her membership coverage.

Your right to request a review
