Hi!

I would like to request if possible to get an old copy of MDR of my late father Nicasio Laureño Abellar for the purpose of legitimation. This is my way that get a public document and proof of father daughter relationship.

His birthday is October 11, 1959
His place of birth is Mandaon, Masbate

Response to your request

We were able to determine that this particular information is already available online and therefore cannot be counted as a valid FOI request. You may visit our website www.philhealth.gov.ph.  Registered members can create an account in our PhilHealth Member Portal, where they will be able to view your records, print their Member Data Record (MDR), pay contributions online, among others https://memberinquiry.philhealth.gov.ph/member/. 

In addition, due to Data Privacy Act, we cannot provide you a copy of MDR. You may also opt to visit any nearest Local Health Insurance Office, and request for a copy of MDR, kindly bring authorization letter and two (2) valid ID's of member and representative.  

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to actioncenter@philhealth.gov.ph. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.

If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
