Hi, I would like to request the PhilHealth claims data in Region VI for all benefit packages. If I can also kindly request the data to include the following information and be disaggregated as follows:

1. by year
2. by patient/member's address (province, and by municipality if possible)
2. by benefit package 
3. by name and address of facility/institution
5. by facility type 
5. by facility ownership (public, private)
6. by claims status, count, and amount paid
