Hello,

I was looking into my records for the contribution. I do understand that SSS, Philhealth and Pag-ibig are mandated and automatically deducted from my pay. However, upon checking my PHILHEALTH contribution , it seems it doesn't tally with my SSS Contribution based on days I was employed. I am particular to the missing PHILHEALTH contribution where I was employed under the company that was acquired and renamed to several name " ContactWorld > ServiceZone > ClientLogic > now SITEL " The Country manager at that time of my exit (year 2006) was Danilo Sebastian Reyes / Dan Reyes.

Response to your request

For validation and verification of your record, may we request you to send an email to actioncenter@philhealth.gov.ph indicating the following informations 1. PhilHealth Identification Number (PIN) 2. Complete Name (Last Name, First Name, Middle Name) 3. Company Name 4. Company Address 5. Employment Period. In addition, kindly attach a copy of your payslips as proof of PhilHealth deduction.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to actioncenter@philhealth.gov.ph. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.

If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
