Hi!

May I know how to correct or update my MDF records?

I personally went to a PhilHealth office on 2016 to update my records with our eldest daughter's birth. I was able to have a printed copy of such MDF with her name and her PIN.

Just recently, we checked through the member portal and her name doesn't appear in my dependents.

May I know how to update/correct this? If membership reference numbers are needed, please let me know to proceed with this query.
