may i request information on the following, to be used solely for research purposes:

1. implementing rules on shock wave lithotripsy, first and second treatment.

2. facilities, both private and public providing shock wave lithotripsy in the whole country. please indicate name of facility, whether health facility is public or private, and please provide addresses of each facility  

1. top 100 procedural cases, according to no. of claims & amount for the whole 2017.
