Hi Philhealth,  Please see below request for information from CY2014-2016. 

- Philhealth locations – Addresses for all Branches and satellites
- Demographic of Philhealth members per area/ branch
- Claims (case rate/ diseases) per area/ branch - Please include the following if available: medical services availed, demographic (except Personal identifiable information)

I would also like to inquire if all hospitals required to have PHIC office/ desk inside the building?
