I kindly request the following information for the completion of my post-graduate thesis. 

Census of PhilHealth cases/claims for years 2016-2020 for the following hospitals located in Camarines Norte, Bicol region. Please include Year of admission, illness type (medical/procedural), out patient / inpatient status, claim status, claims count as well as SUM OF CLAIMS COUNT

Please include SUM OF CLAIMS COUNT for DENIED CLAIMS
