You asked for updating of civil status.

Response to Request

Your FOI request is approved. I enclose a copy of [most] of the information you requested.

Please note that we are only able to provide some of the information you have requested for as <request for information regarding the updating of membership status from single to married. 1. Go to our nearest PhilHealth offices/branches; 2. Submit the accomplished PhilHealth Member Registration Form (PMRF) and attach the Marriage Certificate (bring both the original and a photocopy for verification purposes). Old PhilHealth ID Card (for brides who want to change their surname after marrying in the Philippines) and bring Valid ID for backup purposes.t>.

Your right to request a review
