In your email please indicate the following information: PhilHealth Identification Number (PIN), Complete Name (Surname, First Name, Middle Name), Birthdate, Birthplace, Address, SSS or TIN No. (if any), Name of Dependents and their Birthdate. Also, attach a properly accomplished PhilHealth Member Registration Form (PMRF), and two (2) valid ID's. Kindly read the instructions at the back of the form for other supporting documents.
