Greetings! I would like to request your office the data on premium contributions and the amount and number of claims paid.

Please include the following data:
1. Premium contributions for the years 2000-2008 and 2012-2018 for each member category.
2. The amount of claims paid for each member category for the years 2000-2016.
3. The number of claims paid for each member category for the years 2000-2008.
