You asked for Indigenous People Certificate.

Please note that we are only able to provide some of the information you have requested for as <The LGU Social Worker who administered the issuance of the Certificate of Indigency to the concerned member for submission to the nearest Local Health Insurance Office (LHIO) to facilitate their enrollment and 
appropriate tagging in the membership database.>.
