I would like to request the following information, to wit: 1.) number of accredited PCB1 providers per region per year 2.) number of assigned members per region per year 3.) number of enlisted members/dependent per region per year 4.) number of profiled members & dependents per region per year 5.) amount PFP payment per region per year 6.) number of in-patient cases per region per year for the following illnesses: asthma, acute gastroenteritis, upper respiratory tract infection and urinary tract infection.
